Assessment Visit

iLiv Home Care LLC is a licensed company and is overseen by the Colorado Department of Health and Environment. The Department requires that we perform an assessment visit to ensure that you receive all the information regarding possible services. We want to get to know you in order to create a customized care plan that works well for you and your family.

 

Customized Approach

Our care coordinators take time to get to know you and your family personally. You will be advised who your caregiver is to make sure that you remain comfortable in your own home. We make every effort to assign the same caregiver to you or your loved one when you have a repeating schedule. If for any reason you would like a different caregiver, we will work with you to accomodate the request.

Our caregivers are insured and receive on-going training. We only employ the caregivers who meet our standards for care, compassion, patience, honesty, integrity, and work ethic.

 

List of Services

Once the home care assessment is completed, we create a care plan that includes the services you want. The plan may cover:

• Cooking and light housekeeping

• Laundry and change of bed linens

• Grocery shopping and errands

• Companionship and range of motion exercise

• Bathing, dressing and grooming assistance

• Accompanying to doctor appointments, supermarket, pharmacy

• Assisting with walking and transfer from bed to wheelchair

• Status reporting to family

• Medication reminders

 

If at any time you decide to make adjustments to your care plan, please contact us and we will glady accomodate your request. 

 

Staff

Our caregivers are insured and receive on-going training. We only employ caregivers who meet our standards for care, compassion, patience, honesty, integrity and work ethic. We eVerify and complete background checks on all new hires.
